Save on Your 2014 Medical Premiums With UnitedHealthcare Personal Rewards!

Have you completed your 2013 health actions?

UnitedHealthcare’s Personal Rewards program allows you to earn a premium incentive toward your BSA medical plan premiums for the 2014 plan year. You’ll be healthier and you’ll save money.

How Do I Earn the Premium Incentive?
Your goal is to earn 75 points by completing three health actions, each worth 25 points, before the July 31, 2013, deadline. These health actions include:

1) Health Assessment. To take the Health Assessment, log in to www.myuhc.com, click on the “Health Assessment” link and follow the prompts.

2) Annual preventive exam at your doctor’s office.

3) Biometric screening. Biometric screenings, including BMI (body mass index), LDL cholesterol, fasting blood sugar, and blood pressure, can help assess your risk for health problems, such as heart disease and diabetes.

Don’t Forget the Provider Form! Be sure to print and take the UnitedHealthcare Provider Notification Form with you to your doctor’s appointment.

This form is available on myuhc.com (click “Provider Notification Form” in the Information Center in the lower left-hand corner) and mercerhrs.com/scouting2health  (click the “Tools” link).

Health actions must be completed by July 31, 2013. The completed form, with your doctor’s signature and date, must be faxed to UnitedHealthcare no later than Aug. 19, 2013, in order to earn points for this health action.

When to Use the Member Notification Form

If you do not see credit on your personalized scorecard for a health action you have already completed, simply print the Member Notification Form, follow the instructions on the form, and submit it by Aug. 19, 2013, to receive points for the health action.

Review Your Personalized Scorecard and Track Your Progress

Your personalized scorecard lists your health actions, those you have completed, and total number of points earned, and it identifies other health actions you can take, even though they do not earn additional points. You can view your scorecard online at uhcrewards.healthinsight.com/BSA, and you should also have received a scorecard in the mail in April.

Don’t Wait to Earn Your 2014 Premium Discount!

You and your covered spouse must complete all three health actions by July 31, 2013, with premium incentives reflected in your January 2014 paycheck.

You must complete all three health actions to earn 75 points. If your spouse is participating, you need to earn 75 points each for a combined total of 150 points.

    What if we only complete 1 or 2 of the health actions, will we receive 25 or 50 points?

    What does the 75 points amount to in savings? That information has yet to be made known and yet we have to complete the actions by a certain date.
